POSITION DESCRIPTION: Service Coordinator Associate
RESPONSIBLE TO: Director of Program Services
POSITIONS SUPERVISED: None
DESCRIPTION: This is an associate level position which requires handling routine office tasks like scheduling, data management, maintaining records, managing files, preparing documents/materials for meetings, handling correspondence and communication while supporting the department's daily operations and activities. This includes, but is not limited to, assisting in the development and implementation of individualized service plans, maintaining accurate and up-to-date records and files, coordinating with other service providers and case managers, facilitating communication between providers, monitoring progress and reporting updates or changes to the Service Coordinator, participating in team meetings and performing other administrative tasks such as scheduling meetings and managing correspondence.
RESPONSIBILITIES:
Review all incident reports (medical, behavioral, critical) and ensure appropriate follow up.
Participate in ISP meetings to assist in development of a comprehensive person-centered plan.
Update ISP Programs.
Ensure appropriate persons are notified regarding change in condition, behavior, treatment, or service plans.
Other duties as assigned by supervisor
JOB QUALIFCATIONS:
Strong Communication and Interpersonal Skills: The ability to interact effectively with people served, Service Coordinators, Conflict Free Case Managers, Managers, Directors and other staff members.
Organizational Skills: Keeping track of schedules, files, and records efficiently. Ability to manage multiple projects and tasks simultaneously and provide timely response.
Computer Proficiency: Proficient in Microsoft Office Suite (Word, Excel, Outlook), as well as specialized case management software and other technology.
Attention to Detail: Accurately recording information and preparing documents. Gathering and reporting details from a variety of sources.
Ability to Work Independently and as Part of a Team: Taking initiative while also collaborating with others. Ability to work closely with diverse groups of people in a team environment.
Qualified candidates will possess a Bachelor’s Degree or equivalent in human services, social work, sociology, or psychology, and 1 year of experience working in a similar role preferred. Work experience may be substituted for education.
